Thank you for your interest in our study! Please read this statement before deciding to complete our screening survey. Participation in this research study is completely voluntary. Please read the information below and ask questions about anything that you do not understand before deciding if you want to participate. A researcher listed below will be available to answer your questions. The purpose of this research project is to screen and recruit participants for Dr. Katharine Simon's ongoing study investigating sleep, cognition, and mood in adolescents and adults with Type 1 Diabetes. We plan to enroll 100 youth between the ages of 13 and 18 years and 100 adults between 18 and 45 to investigate the impacts of Type 1 Diabetes.
This screening study involves multiple components:
(1) Screening Questionnaires – You will be asked to fill out this survey that describes your or your child's sleep habits, mental health, physical health and other characteristics. The data will be collected online on this website.
(2) Follow-up: After the completion of this screening survey, you will receive a follow-up email or phone call from us in the next 4-6 weeks detailing eligibility.
(3) Lab Database - We would like to keep your information in our database and contact you about research studies that you or your child/adolescent is eligible for 5 years. You can “opt in" or "opt out” and select a time frame to remain in our participant system. The screening survey takes approximately 15 minutes to complete. Your participation in this screening survey today does not require your participation in any future studies. Your contact information will remain in our participant database for the total amount of time you have indicated. There is a potential risk of anxiety, fatigue, or embarrassment when completing the questionnaire. There may be some risks to being in this study that we don’t know about now. However, by participating in this study, you will learn about whether you are eligible to participate in any of the lab's ongoing research studies. Our research has helped society learn more about the relations between sleep, memory, and health. You do not have to participate in this study. Your participation is voluntary.
Your answers will remain confidential. We will contact you if you are eligible to participate in our study. If contacted, we will use the contact information you provide us. We will identify ourselves as SLEEP Lab research personnel and ask if you are interested in further participation discussion. We will ask you to identify yourself to confirm we are speaking to whomever completed this form. If someone else answers the call or email that is not you, we will discontinue attempts to communicate with you and will not provide any identifying information. We will only attempt to contact you three times to participate before discontinuing our contact attempts.
If you are not identified to be eligible for this study, we will send you a short email thanking you for your time. If you opt in to our data base, we will contact you for potential future studies that you are eligible. If you do not opt in to our participant database, we will delete your contact information and screening survey.
